Photo Upload Tool

Dr. Richard Cummings and his staff welcome the chance to help you achieve your ideal body form. In order for him to give potential patients the best possible service during their initial consultations, he asks that they upload photos of themselves using this simple web form. Those interested in breast augmentation are asked to please include three images, all displaying your body from the chin to the hips: one taken from straight in front of you, one showing you in profile from the left, and one showing you in profile from the right.

Potential liposuction patients should also include three images, but from different angles. A full evaluation will require one photo from straight on with the arms held away from the sides, one from straight behind with the arms crossed over the chest, and one from the side with the arms crossed over the chest (which side does not matter). Each image should show the body from the shoulders to the knees.

These pictures, along with your description of what treatments and procedures you are interested in, will allow Dr. Cummings to form a preliminary plan for addressing your needs before he meets with you face-to-face. Thank you very much for taking the time to complete this step of the process.

To upload the photos, please first save them to your computer's hard drive and be sure to remember their location. Next, click on the "Browse" buttons next to the fields marked 1, 2, and 3 below and use the dialogue boxes that appear to select the image files you saved to your computer. Put one photo file in each of the three fields. Continue filling out the rest of the form and click the "Submit" button. Someone from our practice will contact you upon receiving your email.
